The RFP (Request for Proposals) Templates

for Health and Human Services Pursuant to Chapter 103F, HRS

 

The RFP template is to be used by state agencies to develop RFPs for health and human services pursuant to section 103F-402,  Hawaii Revised Statutes and Chapter 3-143, Hawaii Administrative Rules.

About the template:

  • Most recent revision 4/08.

  • Created in MS Word 2000.  Compatible with all subsequent versions of MS Word and MS Word 98.

  • It is one document divided into sections instead of 5 separate documents.

  • This RFP template will be revised as needed.  State agency personnel should check this website each time an RFP is developed to be assured you have the most recent template.
