How to Apply to Qualify for the Treatment List of Qualified Private Providers

The solicitation for the Treatment List is open once a year.  The next solicitation is scheduled for July 2006.

 

If you are interested in submitting your statement of qualifications you may check back here and/or you may sign up to be notified by e-mail of the next solicitation.   If you are on the SPO provider e-mail list, you will be notified.
