RFP template to be used by purchasing agencies

Effective October 1, 2004, state agencies began posting RFPs for health and human services to the State Procurement Office's (SPO) Request for Proposals website (RFPW).   This site contains all RFPs for the Executive Branch. 

Note that requests for information are on the Procurement Notices Website.

For each RFP in which you may have an interest, it is especially important that you register your interest with the respective purchasing agency to be apprised of any changes/addenda that are issued.  You may use the RFP Interest form (MS Word format) or  e-mail the information to the contact person for the RFP.

1. Download the form by right clicking and selecting "save target as;" 

2. Complete the form on your computer; and

3. Mail, e-mail or fax the completed form to the contact person listed in the RFP.

All RFPs are in PDF format. To view the RFP: Click on the document(s) under 'Get the RFP.' To download the RFP: Click with your right mouse button and choose 'save target as.' You will be able to open and navigate through the document more quickly if you download it first.

There is also an archive of older closed RFPs posted on a voluntary basis by state agencies between 10/03 and 8/04.  Note that these RFPs are unofficial and do not include any addenda or changes that may have been made subsequent to the initial posting.
